What will you be asked to do?

Call us at (413)577-1823 or send an email to k9tumor@umass.edu
Complete a questionnaire regarding the health and history of your dog.
Allow us to obtain your dog’s pathology report, if you have one. We will send you an automatic follow-up survey.
Agree to an annual follow-up survey lasting for up to 10 years.
